Il doit son nom à l'instrument chirurgical appelé lancette (en anglais : lancet), sorte de scalpel. Le rédacteur en chef actuel s'appelle Richard Horton.
Le premier numéro date du 5 octobre 1823 et a été lancé par Thomas Wakley qui éditera la revue jusqu'à sa mort[1].

Le site web thelancet.com a été lancé en 1996 avec l'intégralité des articles en ligne depuis mars 1998 (sur abonnement pour les plus récents).
