La clozapine (Leponex, Clozaril) est un antipsychotique atypique dérivé du dibenzodiazépine utilisé dans le traitement de la schizophrénie qui agit par régulation des fonctions cognitives.

Bonne affinité pour les récepteurs D4, ce qui le différencie des autres antipsychotique. En général, moins d'occupation des récepteurs D2 et D1, ce qui empêche les troubles extra pyramidaux (sauf à hautes doses), et les dyskinésie tardives.

Améliore sensiblement les symptômes négatifs des psychoses et les cadres rebelles aux autres neuroleptiques. Améliore la fluidité verbale, la mémoire immédiate et différée et d'autres fonctions du lobe frontal chez les schizophrènes [citation nécessaire].

Il a un effet très intéressant consistant en la réduction de idées suicidaires de près de 84%.

Présente une biodisponibilité de 50%, une union à protéines de 95% (attention aux interactions médicamenteuses d'ordre pharmacocinétique), il est très métabolisé, et présente une demi-vie de 6/30h.

Ses effets secondaires peuvent être :

  • agranulocytose allergique (80% dans les trois premiers mois) réversible par stimulation des colonies granulocytaires.
  • troubles gastro-intestinaux
  • tachycardie par blocage vagal
  • incontinence
  • hypersalivation
  • prise de poids
  • altération de l'électrocardiogramme dans 25% des cas, avec un risque convulsif.
