Syndrome de sécrétion inappropriée d'hormone anti-diurétique


Le syndrome de sécrétion inappropriée d'hormone anti-diurétique (SIADH) est un syndrome souvent rencontré dans les hôpitaux, en particulier chez les patients hospitalisés pour des problèmes au système nerveux central (SNC). Comme le nom l'indique, c'est un syndrome caractérisé par une libération excessive d'hormone antidiurétique (ADH ou vasopressine) de l'hypophyse postérieure. Il en résulte une hyponatrémie, et parfois une surcharge en liquides.

Physiopathologie

Normalement l'hormone anti-diurétique (ADH) contrôle la concentration osmolaire de l'organisme. Elle maintient une concentration osmolaire stable par l'intermédiaire du volume d'eau contenu dans le corps humain. L'ADH entraine une rétention d'eau 'pure' au niveau du tube contourné distal au niveau du rein. L'eau retenue, dilue les osmoles en trop qui ont stimulé la sécrétion d'hormone anti-diurétique.

Les mécanisme de sécrétion d'ADH en dehors de ses contrôles habituel sont imparfaitement élucidés. Des tissus deviennent capable de sécréter une hormone semblable à l'ADH en grande quantité ; c'est le cas des cancers et infections broncho-pulmonaires.


Clinique

Il n'y a pas de signe spécifique de ce syndrome. Les signes majeurs sont ceux de l'hyperhydratation intra-cellulaire ou hyponatrémie ; nausées, vomissements, troubles du comportement et confusion. Dans les cas d'hyponatrémie sévére, le coma et les convulsions sont fréquentes.
