Mir Mahmoud Hotaki est le fils de Mirwais Khan Hotak de Kandahar (Afghanistan).


Après avoir pris le pouvoir, il envahit l'Iran Safavide en 1722 et marche sur la capitale, Isfahan. Cette même année, l'armée afghane bat les Safavides à Gulnabad en 1722 et établit ainsi leur pouvoir sur l'Iran. Après cela, le souverain perse Shah Husayn est obligé de se rendre et abdique. Mahmoud fait tuer le 8 février 1725 les six frère du Chah, tandis que le fils d'Huseyin, Tahmasp II, s'échappe et continue la lutte. Après la mort de Mahmoud en 1725, son cousin Ashraf Ghilzai lui succède.
